2020-2021 Registration Forms & Requirements - for NEW students only!

  • The 2020-21 NEW REGISTRATION will open on February 24, 2020


    Registration at Loma Prieta Joint Union Elementary School District (LOMA) must be completed online. Please submit your online registration before submitting the hard copy packet. After submitting the online portion of the registration packet, parents must complete and submit in person the following documents to complete the registration process. The registration of your child in LOMA is not complete until both online and hard copy registration forms are validated.

    Parents may download and print the following forms or pick up a packet at the school or district office. Please click the Information Link on the right that corresponds to your child's grade level for additional information and to download the appropriate forms.

    • 01 - Proof of Residence for Enrollment
      02 - Report of Health Examination for School Entry
      03 - Oral Health Assessment Form (ONLY for NEW students entering public school for the 1st time)
      04 - Kindergarten Questionnaire (Kinder ONLY)
      05 - Medication Permission Slip  (Students requiring medication during school time ONLY)
    • 06 - Santa Clara County Tuberculosis Risk Assessment for School Entry - requires Health Care Provider's signature

    NEW K-8 registration hard copy packets will be accepted at the LOMA District Office* beginning Monday, February 24, 2020 at 12:30 pm.

    District office hours are between 9 am and 1 pm Monday through Friday during the ordered Shelter in Place.  Once the SIP has been lifted, we'll return to our regular house of 8 am to 4 pm.
